Position Overview

The Referred Care Coordinator provides comprehensive support for patient referrals and specialty care coordination. This role serves as a primary point of contact for patients navigating referred care services, helping ensure appointments, referrals, records, and follow-up care are coordinated efficiently and accurately.

The Referred Care Coordinator works closely with patients, providers, care teams, referral vendors, and community partners to reduce barriers to care and support positive patient outcomes.

Key Responsibilities

Serve as a primary contact for patients regarding referral appointments and referred care services

Coordinate referrals with specialty providers, healthcare facilities, and external vendors

Assist patients with scheduling appointments and navigating referred care processes

Help connect patients with insurance, benefits, travel, and support resources when appropriate

Monitor referral workflows and maintain timely follow-up on pending referrals and appointments

Track referral status, appointment completion, and receipt of consultation reports and medical records

Communicate with patients, providers, and external healthcare organizations to ensure continuity of care

Identify and address barriers that may prevent patients from completing appointments or receiving recommended services

Support administrative processes related to laboratory, imaging, and specialty care results

Maintain accurate documentation within the electronic health record and referral management systems

Prepare patient correspondence and provide exceptional customer service throughout the referral process

Qualifications

High school diploma or GED required

Three years of experience working with the public in a healthcare, medical office, patient services, or related setting; equivalent combinations of education and experience considered

Knowledge of healthcare operations, patient referral processes, and medical office practices preferred

Familiarity with Medicaid, Medicare, commercial insurance, and healthcare reimbursement systems preferred

Knowledge of HIPAA and healthcare privacy regulations

Strong organizational, multitasking, and time-management skills

Excellent communication and customer service abilities, including comfort with high-volume telephone communication

Experience using electronic health records and database systems preferred

Ability to prioritize competing responsibilities in a fast-paced healthcare environment

Work Schedule

KANA’s standard work schedule is 37.5 working hours per week, consisting of 7.5 paid working hours per day plus a one-hour unpaid lunch break (8.5 hours on site per day).
