POSITION SUMMARY
Responsible for the Care Coordination for all recipients of IDD, CCMC, and APDD Medicaid Waiver Services in our region. Oversees the Assistant DD Case Manager. Ensures the continuum of services provided to developmentally disabled individuals and their families in the Northwest Arctic Borough and Point Hope. This position reports directly to the Senior and Disabilities Services Director.
PRINCIPAL DUTIES AND RESPONSBILITIES
Responsible for implementation of Care Coordination Services for all recipients of Medicaid Waiver Services within the region.
Assures the creation, currency, and on-going implementation of the Plan of Care (POC) for all recipients of Medicaid Waiver Services, maximizing the use of community resources.
Assures the compliance of all State of Alaska Senior and Disabilities Program regulations and Medicaid Waiver Services requirements.
Responsible for personnel issues including hiring, disciplinary actions and evaluation of Assistant Case Manager(s).
Conducts staff meetings to ensure the continuum of Care Coordination services for consumers receiving HCB waiver services.
Conducts case reviews to assess participant’s progress and satisfaction in meeting goals and objectives established in the plan of care.
Reviews and updates the assessment and plan of care annually or when a change in the recipient’s condition warrants it.
Coordinates with the providers of services to ensure the sharing of needed documentation such as approvals of level of care, plans of care, eligibility for waiver participation and received prior authorizations for billing purposes.
Assist consumers and their families with Medicaid, SSI and public assistance applications.
Monitors all documentation, as it pertains to clients, is current, sufficient and appropriate.
Maintains Medicaid Waiver Care Coordination certification with current provider number.
Contacts recipients on regular basis, traveling to villages as needed for face to face meetings.
Obtains, Non-violent Crisis Intervention training, and any other training experiences deemed necessary.
Assists in educating the community and other human service providers on the special needs of the developmentally disabled. Will ensure program is represented in Health Fairs.
Works cooperatively with the Northwest Arctic Borough School District, Division of Family and Youth Services, Vocational Rehabilitation, Community Health Aides and other providers to assure a continuum of services for the disabled.
Job Qualifications:
MINIMUM REQUIREMENTS
Have 2 yrs of post-secondary education in a human services field and 2 yrs working in the field of developmental disabilities: or 5 years’ experience working with the individual with developmental disabilities. Have a minimum of 2 years of supervisory experience. Obtain certification as a Care Coordinator for the IDD, APDD and CCMC Medicaid Waiver Services Program. CPR/1

Aid certification. Ability to lift 40 lbs. Ability to accurately and efficiently use standard office computer equipment and software, faxes, copiers, telephones, and 2-way radios. Current Alaska Driver’s License with a good enough driving record to be covered by Maniilaq’s insurance. A willingness to fly on small aircrafts for village outreach travel required. Familiarity with the Northwest Arctic Borough, the culture, its people and the ability to speak Inupiaq is desirable.
