Overview

The Case Manager position is responsible for working with youth (ages 12-18) with substance abuse and/or emotional and behavioral disorders and their families in VOA Alaska’s ARCH Program. Their goal is to facilitate their movement toward greater self-sufficiency and successful achievement of their goals. A Case Manager will mentor, educate, and spread the message of recovery to individuals who are new in their recovery journeys. A Case Manager will provide emotional and social support to VOA clients, and model healthy supportive relationships that encourage sobriety and living a life in recovery.

VOA Alaska is a youth-serving organization providing prevention, early intervention, treatment, and recovery supports to youth and families struggling with mental illness and addiction. Essential Functions, Duties & Responsibilities

Care Coordination & Case Management

· Facilitate administration of needs assessments for all youth enrolled in the ARCH program.

· Provide intensive case management services as directed

· Provide a minimum of 20 hours of direct client care weekly.

· Support clients in accessing community resources including housing, employment, education, transportation, healthcare, recreation, and recovery-oriented activities.

· Facilitate Medicaid Travel

· Develop supportive and professional relationships with clients while maintaining appropriate boundaries and ensuring client dignity, safety, and privacy.

· Utilize strengths-based, trauma-informed, and culturally responsive approaches in all client interactions.

· Provide individual support sessions, weekly groups, and recovery-focused coaching as needed.

· Facilitate warm handoffs and coordination of care with internal and external service providers.

· Communicate with guardians, stakeholders, treatment providers, and community partners as assigned.

· Model healthy coping strategies, life skills, and positive lifestyle changes that support recovery and independence.

· Participate in treatment team

Intake, Discharge & Program Operations

· Coordinate and facilitate client intakes, including transportation, documentation completion, and some orientation, and admissions processes.

· Manage discharges and transfers, ensuring completion of discharge checklists, medication coordination, belongings documentation, and clinically appropriate discharge planning.

· Oversee treatment and medical passes

· Facilitate client allowance distribution and support appropriate access to personal funds as needed.

· Support continuity of care through coordination with families, guardians, and community providers during admissions, transitions, and discharge processes.

· Maintain accurate records related to client movement, transportation, medications, and program participation.

Documentation & Administrative Responsibilities

· Complete timely and accurate documentation for all services provided in compliance with agency, state, and federal standards.

· Prepare reports, staffing summaries, and additional documentation requests as assigned by leadership.

· Support program administrative functions that directly impact client care, safety, and service coordination.

· Assist with data tracking, scheduling coordination, and communication related to client services and program operations.

Compliance & Safety

· Maintain client confidentiality and safeguard protected information in accordance with HIPAA, 42 C.F.R. Part 2, agency policy, and applicable regulations.

· Safely transport clients using agency vehicles as required.

· Participate in recreational, community-based, and recovery-oriented activities with clients as appropriate.

· Maintain awareness of safety protocols, crisis response procedures, and mandatory reporting obligations.

· Perform other duties as assigned.

Knowledge, Skills and Abilities

· High School Diploma or equivalent.

· Has qualifications to be a Qualified Addiction Professional or Peer Support Specialist as defined by the State of Alaska. If not current, can be achieved within 3 years of hire.

· Qualified Addiction Professional (QAP): Chemical Dependency Certification level II (CDC II) and/or National Certificated Addiction Counselor I (NCAC I)

· Peer Support Specialist (PSS): The ability to self-identify as someone who has lived experience of recovery from mental illness and/or addiction.

· A current and valid driver’s license

· Can qualify for VOA’s automobile insurance and/or proof of vehicle insurance coverage

· Reliable transportation

· Excellent writing and communication skills

· Ability to work flexible hours (including days, evenings, weekend hours and Holidays.) Ability to safely transport clients in agency’s vehicle if required.

· Ability to participate in recreational activities; such as hikes, and/or indoor/outdoor events with clients.

· Bilingual in Spanish preferred.

· Ability to provide current credentials and if not current, willing to work towards certification requirements and obtain QAP within 3 years of hire. · Works in office areas. Interacts with staff, consultants, and outside vendors. May be subjected to interruptions throughout the workday.

· While performing the duties of this job the employee is frequently required to sit; use hands to finger, handle, or feel; and talk or hear. The employee is occasionally required to lift up to 25 pounds. The vision requirement includes close vision; consistent sighting of clients.
